2/17/2024 EnchantaVaughn Approved Behavior: Client greeted QMHP by saying how are you doing today and hugging her. Mom was observed sitting quietly on the couch watching a movie with her other daughter. Client appeared to be in an excited and upbeat mood as evidenced by her smiling and initiating a conversation with QMHP. Client expressed that she was feeling good and was going to have a good day. Client displayed productive behaviors and was very persistence throughout the session. Behavior: Client shared positive thoughts about her day and continued making good choices, which were supported by QMHP and mother. The client was supported for displaying an understanding of the social skills that were reviewed in previous sessions. Client verbalized, âIâm trying to be a nice personâ in response to QMHPâs questions. Client was able to stay focus during the session. Client was polite and courteous of others. Client displayed appropriate behaviors while interacting well in the home. Intervention: QMHP supported client displaying positive social interaction within the home environment. QMHP praised and recognized client for her hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of her therapy. QMHP provided simple praises to client for her commitment to her progress. Intervention: QMHP modeled proper positive behavioral expectations for the client to encourage additional layers of support at home. QMHP implemented individualized behavior management techniques with client. QMHP processed with client about psycho educational activity while assisting with directions. QMHP explained the importance of the activity. QMHP provided feedback to client in regard to questions asked about the activity. QMHP explored clientâs responses and provided appropriate feedback. Response: Client avoided distractions in social setting, while utilizing anger management skills. Client displays leadership skills by helping others and being good to her family. Client engaged in reflective thinking and expressed her feelings to QMHP. Client was in a pleasant mood after feedback was given. Client was acceptive of encouragement and decided to listen to her mother and do the right thing. Client was seen having a good time. Client was able to identify the role she plays in each situation and continues to comply with authority rules and regulations. Response: Client displayed respectful, non-aggressive behaviors in the second half of session. Client appeared to be heavily influenced by others who are more direct and confident since she is not as direct or confident. Client acknowledged interventions and verbalized clear understanding of each intervention. Client communicated the do's and don'ts of home rules so that she shows more commitment to her choices. Client remembered to apply herself and utilize her strengths to focus on meeting the goals listed on her ISP.
